Home Health Nurse Supervisor / Per Diem Nurse (On-Call)

We are seeking a dedicated and skilled On-Call Nurse / Nurse Supervisor to join our team on a per diem basis. In this role, you will oversee nursing care, ensuring high-quality, patient-centered care that adheres to physician-prescribed treatment plans and the American Nurses Association (ANA) Standards of Care. You will play a critical role in assessing, diagnosing, and treating health conditions, all while upholding our agency’s policies and ensuring patient safety.

Qualifications:
Active Registered Nurse (RN) license in New Jersey.
Minimum of 1 year of nursing experience, with a preference for home care.
Successful completion of the Home Care Exam.
Valid driver’s license, auto insurance, and professional liability insurance.
Availability for on-call hours, including readiness for home visits.

Key Responsibilities:
Provide safe, competent, and compassionate nursing care.
Effectively communicate health status and treatment plans to patients, families, and caregivers.
Administer prescribed medications and maintain accurate documentation of patient care.
Collaborate with healthcare teams to develop and implement comprehensive patient care plans.
Assess the biophysical, psychosocial, and educational needs of each patient.
Respond promptly to changes in patient conditions and communicate with the Director of Nursing and physicians as needed.
Uphold patient confidentiality and adhere to infection control practices.
Monitor and manage patient supplies and equipment, ensuring functionality and safety.
Plan and deliver supportive and restorative care.
Coordinate necessary community and social services to support clients and caregivers.
Ensure timely submission of all required documentation, including time cards and patient records.
